Some services provided by the Practice are not covered under our contract with the NHS and therefore attract charges. Examples include the following:

• Medicals for pre-employment, driving requirements (HGV, PSV) etc.
• Insurance Claim forms
• Firearm/shotgun applications
• Prescriptions for taking medication abroad
• Reports including lasting power of attorney
• Holiday cancellation certificates
• Fitness for sports activities, including charitable events

The fees we charge are based on the British Medical Association (BMA) suggested scales and our clinical administration team will be happy to advise you about specific services and fees. All fees are to be paid in advance, before the report or certificate can be passed to the GP to complete or an appointment booked where a physical examination is needed.

Please note that in many cases a print of part of your medical record may be sufficient. Details can be printed via access to your record on the NHS App or please send a request to our clinical administration team via the practice website. There is no charge to provide one printed copy – there may be a charge for a duplicate copy or subsequent requests for the same information.
